How to Purchase

1. Fill out the order form by selecting the gift cards you want and the quantity.

   • You pay the face value of the card.
   • LAOH Division 2 earns a percentage back.
   • Be sure to total your order.
   • There is no activation fee for VISA cards.

2. Payment is due at the time of order.

   • Payment may be made by cash, money order, or check.
   • Make checks or money orders payable to LAOH Div 2.

Submitting Orders

Contact Kathy Scheid at 631-379-7933 to arrange submission of your order.

Orders may also be left in the lounge. Place the order in an envelope marked:

LAOH Gift Card Order – Attention Kathy

Let Kathy know that the order has been left there.

Once an order is received, customers will be contacted with pickup details.

2026 Order Deadlines

There are two opportunities to purchase gift cards this year:

• Early Order Deadline: October 26, 2026
  In time for Thanksgiving

• Holiday Order Deadline: November 30, 2026
  In time for Christmas

Orders cannot be accepted after these dates.
